Specialized Adult Day Range of Services

Included in your loved one’s day is the following range of services:

  • Advocacy and assistance in expediting the admissions process

  • Assessment of need, detailed plan of care, and case management

  • A full day of stimulating and interesting activities for your loved one

  • A full day of supervision from a licensed nurse and caring, trained staff

  • Emergency care, as required

  • Medication management and administration

  • Regular monitoring of vital signs

  • Quarterly progress reports to the patient’s doctor

  • Blood glucose level monitoring for patients at risk

  • Dressing changes

  • Physical therapy assessments and referrals

  • Coordination of special diets, such as diabetic, no added salt, puree

  • Hands-on assistance in the bathroom

  • Assistance with grooming and hygiene

  • Assistance with walking and transferring from wheel chair to chair

  • Hands-on assistance with meals

  • Coordination of lunch and snacks

  • Coordination of transportation

  • Weekly caregiver support groups

  • Access to ACC’s educational resource library

  • A subscription to ACC’s quarterly educational publication, Alzheimer’s Community Care Magazine
     


The Day Care program is a “dementia-specific” program due to these factors:

  • Applicants must be diagnosed or suspected to have Alzheimer’s disease or a progressive, irreversible condition causing dementia

  • Alzheimer's Community Care is aware of the special needs of people with a condition of this type and training is provided on how to respond to those needs.

  • The staff to patient ratio is maintained at 1:5; 1:3 with volunteers.

  • Activities, projects, and events that result in success with people with dementia are conducted within a structured, daily calendar.

  • Additional support is available and is coordinated to make living with Alzheimer’s a little easier.
