Standards of Care

  1. The Specialized Alzheimer’s Adult Day Service Center only provides services to patients 18 years of age or older who have been diagnosed with a dementia-related disorder.

  2. Patients and/or caregivers are strongly encouraged to seek diagnosis from a licensed neurologist or Memory Disorder Center.

  3. The safety and well-being of our patients and caregivers is our first priority. A safe environment is maintained at all times, and emergency procedures, disaster plans and drills are reviewed and conducted regularly throughout the year.

  4. “Hands-on” care is provided for patients requiring assistance with activities of daily living (i.e. toileting, dressing, feeding, transferring, ambulating).

  5. Because of the clinical nature of the disease process, the ongoing involvement of a professional nurse is paramount. Each Specialized Alzheimer’s Adult Day Service Center employs a minimum of one full time Licensed Practical Nurse (most centers employ two LPN’s) and ongoing monitoring by a Registered Nurse.

  6. Ongoing communication with each patient’s caregiver and physician is a critical component of care. Each patient’s physician receives an update on the patient’s involvement in the Specialized Alzheimer’s Adult Day Service Center quarterly, at minimum. Individualized Care Plans are reviewed with caregivers regularly.

  7. A ratio of one staff to five patients is maintained throughout the day.

  8. Therapeutic activities are available for patients throughout all hours of operation and serve as the foundation for treatment in the Centers as they reduce isolation, minimize depression and enrich the lives of both patients and caregivers.

  9. All Alzheimer’s Community Care Staff and Volunteers complete a comprehensive dementia certification course to gain a better understanding of dementia-related disorders, as well as the need for dementia specific services and resources.

  10. All therapeutic activities can be adapted for patients throughout all stages of the disease process and are physically, cognitively and socially stimulating.

  11. Therapeutic activities are designed to be successful experiences which focus on the patient’s strengths rather than deficits.

  12. Therapeutic activity programming includes opportunities for caregivers and family members to participate in special events with their loved one throughout the year (ex. Thanksgiving Celebrations, Caregiver Appreciation Events, Cookouts, etc.).
     
  13. Therapeutic activities shall meet the varying interests and cultural lifestyles of the patients that we serve.

  14. Therapeutic activities shall provide opportunities for self expression through creative endeavors such as art, music and movement.

  15. Therapeutic activities shall provide patients with the opportunity to connect with the community through regular outings and special events.

  16. All services are community based and strategically located throughout our three- county service area to ensure accessibility for patients, caregivers, staff and volunteers.

  17. Alzheimer’s Community Care collaborates with local faith-based organizations to provide cost effective Specialized Alzheimer’s Adult Day services in the community.

  18. All Centers are located on the first floor of every building we occupy.

  19. Specialized Alzheimer’s Adult Day Service Center operational hours should meet the needs of our caregivers therefore each location is open every weekdays for a minimum of nine hours per day (most sites are open ten or more hours per day).
