Alzheimer's Community Care:
Helping Patients and Caregivers Every Day
The programs and services we provide to the communities we serve are made successful only by the passion, dedication and commitment from staff and partner agencies. Our caregivers say it best, right here, in their own words.
Adult Day Centers
Dear Joe,
Words cannot express what this center means to our familiy. Alzheimer's Community Care has given my mother purpose and meaning. She is motivated to get up in the morning. The activities stimulate her and the people are her second family. I cannot imagine where my mother would be without it. I am very thankful for the people and Alzheimer's Community Care. --A. Hodge-Jefferson, December 17th, 2009
Danielle,
Just wanted to let you know what a pleasure it was to meet you. My husband was able to start at the day center last week. He adapted to it with no issues. He even said that he had fun. What a relief it is to me. Hopefully he will be able to attend all five days before too long. That will be a big stress relief for me. --J. Brown, June 14th, 2010
Edna, Joe and Staff:
We would be lost without the invaluable care and attention your team provides our Mom. May you continued to be blessed with a caring and loving spirit! Hoffman siblings
Jo Ellen and Edna:
You and your staff have done a wonderful job of taking care of my father and also been an amazing source of support to my mother. This is just a small token of our family's appreciation for all your great work. N. Ellery, November 10th, 2009
Ron:
My family would like to thank you for everything you did for our Mother while she attended the day center. It has been a very emotional past few months for our family making sure her needs, health and well-being are addressed. I must add that she is doing very well.
Everyone at the center was truly fantastic with our Mother. I enjoyed the family gatherings, such as Thanksgiving. You and your staff met our needs so we could go to work. I can't express enough to your entire staff how great you all are. G. Lewis and family, March 4th, 2010
Dear Kathy:
Words are certainly inadequate to express to you our appreciation for you and your staff for the extreme difference you have made in the quality of our lives. When Joe is not attending the day center, he has little interaction with his surroundings, minimum socialization and practically no physical activity. Even though we have tried to provide these things at home, it has not been successful. At your center, I see him talking, laughing, singing, exercising and appearing very happy.
In addition to what you are doing for Joe, you have enabled me to have time to do the myriad tasks required to be a caregiver and also a time of respite from the 24-hour-a-day care. Your staff should be commended on the respect you show each patient; each of you have shown a desire to be as helpful as possible and exhibits a caring and concerned attitude toward families even under tough circumstances. Your help is enabling me to keep Joe at home for much longer than would otherwise have been possible. D. Sanders, March 9th, 2010
Case Management
Dear Danielle,
You helped me so much! Thanks for making life financially and emotionally easier. You brought sunshine to my husband and myself! --B. Goldstein, May 27th, 2008
Dear Darlene,
Just a short note to share with you how impressed I am with your organization.
As with so many caregivers, I was lost in making social, family and financial ends meet with my situation. Only six years ago, I lost my wife to breast cancer and until now, have never had any kind of assistance. While trying to riase my children in the way I promised my wife, I learned I needed to expand my family and financial role even more with my Mom's diagnosis of Alzheimer's disease. I've had to provide 24-hour care and housing. This compounded my already overwhelmed feelings of despair and helplessness. Finally, last year while attending church, your organization held a meeting about Alzheimer's disease and the help available. It was truly divine intervention.
Since Mom has been accepted into the program, I have been able to find the time to open my heart and feelings to my other family members. Your program is everything and more than was promised. Joe Pisz, our case manager, has gone out of his way to make sure my mother has everything she neds. He exemplifies how quality care is not just a phrase, number or a statistic. Rather, he reflects the true caring and dedication money alone cannot buy. Thanks, and keep up the great work! M. Steetle, November 3rd, 2008
Family Nurse Consultant Service
A caregiver in Delray Beach was experiencing serious financial problems and was unable to pay his condominium fees and mortgage. After calling Alzheimer's Community Care, he spoke with a Family Nurse Consultant who enlisted help from a partner agency, 211, to find available help. The Family Nurse Consultant coordinate for the the caregiver to receive assistance with housing fees, food stamps and mortgage. The Family Nurse Consultant also found funding via an available aid program so that his loved one could attend a day center. The program at first did not cover in-home care during the evening, but after working with the Family Nurse Consultant and the program manager from the day center, the aid program provided for in-home personal care for the patient.
Caregiver Support
To all:
Thanks so much for a wonderful Thanksgiving dinner at the center! As always, it was the best of everything. You all are just the greatest. L. and S. Couzman
To the staff:
Although it's been many years since my mother has been in your care, I will never forget the kindness that your team showed her. It's a great help to the caregiver to have a safe and loving place to take a loved one. Mother has passed and she is missed, but I'm comforted knowing she had a good life and that she's in a better place. Please accept my sincere thanks for all that you do. N. Andrews, March 29th, 2010
