Alzheimer’s Community Care Wins 2025 Florida Blue Sapphire Award

Alzheimer’s Community Care (ACC) has been honored with the 2025 Florida Blue Foundation Sapphire Award for its Family Navigators program. This initiative offers critical support, resources, and interventions to caregivers of individuals with Alzheimer’s and related neurocognitive disorders, enhancing home-based care and improving patient outcomes.
