The Alzheimer’s Community Care’s ID Locator Service provides individualized solutions and education to ensure the safety and security of identified patients with Alzheimer’s disease or a related neurocognitive disorder.  The Organization has a long standing partnership with Law Enforcement in assisting with wandering events within our three service territories of Palm Beach, Martin, and St Lucie counties.

Through a professional, dementia-specific evaluation by a Family Navigator, the patient and caregiver are deemed eligible for the service and recommended safety tool.  Each proven safety tool is provided at little or no cost.  The benefits outweigh the trauma and anxiety an elopement event can cause. and through support, guidance and education provided by the ID Locator Team, the caregiver will have peace of mind and confidence in knowing what to do in such an event.

Proven technology such as Radio Frequency or GPS, assist rescue personnel in tracking a patient’s location within minutes rather than hours.  Prompt location of a patient is crucial as 80% of those missing for more than 72 hours will not survive an event.
