Support for those Living Alone with Alzheimer's Disease

Alzheimer’s Community Care is proud to offer special attention for individuals with Alzheimer’s Disease or Related Disorders (ADRD) who live alone.

Alzheimer’s Community Care has several ways to support the needs of an Alzheimer’s patient that lives along. The hope is to promote their independence, quality of life, safety and well-being.

The goal of this program is:

  • Increasing the potential for the person living alone to remain safely at home
  • Identifying and coordinating an informal network of care partners and support for the person
  • Increasing the opportunity for socialization, cognitive stimulation and connectivity

Benefits & Services:

  • Family Navigator: Quarterly, in-home visits and individualized care plans
  • Phone Support: Regular telephone calls and one-on-one attention from a dementia-specific trained peer
  • Resources & Services: Connection to patient-appropriate resources and referred program

- Diagnosis of Alzheimer’s disease

- Live alone
