Alzheimer’s Community Care is dedicated to promoting and providing community-based, family-centered care for patients and their caregivers living with neurocognitive disorders, through the belief that where there is help, there is hope.
Over the years, the Organization has been the driving force in advocating and establishing much needed legislation in the state of Florida.
A Specialized Day Center providing dementia-specific care and therapeutic activities to the patients. Other services include Family Navigator and ID Locator Service.
Our Family Navigators provide caregivers with the support, tools, and resources needed to continue providing home-based care for their loved ones.

Our Education Department provides the latest education on Alzheimer’s disease and related neurocognitive disorders for patients, caregivers, staff, law enforcement, students of health professions and community health care partners. We also strive to monitor, support and promote the Organization’s accredited model for specialized, disease-specific care.
