Alzheimer’s Community Care is dedicated to promoting and providing community-based, family-centered care for patients living with neurocognitive disorders and for their caregivers through the belief, where there is help, there is hope.
Over the years, the Organization has been the driving force in advocating and establishing much needed legislation in the state of Florida.
A Specialized Day Center providing dementia-specific care and therapeutic activities to the patients. Other services include Family Navigator and ID Locator Service.
Our Family Navigators provide caregivers with the support, tools, and resources needed to continue providing home-based care for their loved ones.

We also strive to monitor, support and promote the Organization’s accredited model for specialized, disease-specific care.
