About Alzheimer’s Community Care

Alzheimer’s Community Care is dedicated to promoting and providing community-based, family-centered care for patients and their caregivers living with neurocognitive disorders,

Our History

Alzheimer’s Community Care (ACC) was founded in 1996 by a group of Florida residents who were concerned about the growing number of people affected by Alzheimer’s disease and related neurocognitive disorders.

They strongly believed in establishing a community-based organization for dementia patients and their caregivers that would provide them with an environment explicitly planned for their safety and security, health, and well-being, while preserving their dignity throughout the disease process.

Today, Alzheimer’s Community Care is the largest provider of community-based, dementia-specific services in Florida. The organization has 7 core services, over 80 full-time care and operational professionals serving the organization, and 11 Specialized Alzheimer’s Licensed Day Centers.

Why We Do What We Do

There are just under 580,000 people living with Alzheimer’s disease in Florida, and nearly half of them are undiagnosed.

There is no cure for Alzheimer’s disease, so understanding risk factors and early diagnosis is key in preventing dementia and slowing progression.

More than 840,000 caregivers in Florida provided 1.3 billion hours of unpaid care in 2024.

Our services provide much needed resources, respite, and peace of mind for family caregivers and enable their loved ones to live at home for as long as possible.

Our Core Services

Family Navigators

Family Navigators guide caregivers and patients through the disease process, offering the support, tools, and resources needed to keep families together in the community for as long as possible.

Specialized Alzheimer’s Adult Day Centers

Specialized Alzheimer’s Adult Day Centers afford respite to caregivers, allowing a break from their caregiving duties and providing a therapeutic environment for their loved one to remain active, engaged, safe, and secure.

Case Management

Case Management services work closely with families to successfully connect them to outside agencies offering critical financial assistance.

ID Locator Program

The ID Locator Program addresses the dire reality that a person with a neurocognitive disorder will likely wander from their supervised setting and become lost. Our safety tools and training are provided at no cost to families to aid in the safety and security of their loved one.

24-Hour Assistance Line

A 24-Hour Assistance Line provides access to one of our highly trained professionals to help caregivers with non-medical urgent needs that cannot wait until the next business day.

Education Department

Our Education Department delivers community presentations and professional training to law enforcement, health care professionals, and students. The department also monitors and supports the ACC’s accredited model for specialized, disease-specific care.

Advocacy

Advocacy for the resources and services needed for families is at the heart of our mission. We inspire legislative change in Tallahassee and are proud to have led the workgroups for theFlorida Silver Alert and Lost on Foot protocols, and to elevate the standard of dementia care provided to patients through the Specialized Alzheimer’s Adult Day Care Act.

How to Support ACC

We are grateful for your time, talent, and charitable donations.

Day Center Volunteer Opportunities

Lead an activity for the patients (art, music, holiday)

Be a patient companion by assisting them with their daily activities

Participate in beautification projects, such as gardening or painting

Join a Committee

Advocacy & Community Outreach Committee

  • Educate local/state elected officials about ACC through special day center tours and advocacy events
  • Participate in annual lobby day inTallahassee
  • Actively promote key events, such as pre and post legislative session debriefings
  • Attend ACC advocacy trainings
  • Assist staff with planning and promoting community education events

Development Committee

  • Assist development staff with prospective donor and sponsor identification
  • Host friendraisers
  • Act as an ambassador in the community and make introductions between staff and potential sponsors/donors
  • Participate on fundraising event subcommittees
  • Make an annual gift to ACC

NextGen CARES

NextGen CARES (Committed to advocacy, resources, engagement, support) is open to young professionals who are invested in ACC’s mission and services and want to support our success through philanthropy and volunteerism while growing their personal and professional networks.

Members are required to commit to a minimum monthly recurring contribution of $50.
